If this is you

You've been doing this for years.
The results don't match the effort.

You track your food. You go to the gym. At your last physical, your doctor told you to eat better and exercise more. This is advice you've been following since your late 30s.

The scale moves a few pounds, then comes right back. Your energy is lower than it was five years ago. You've tried structured programs, cut carbs, tried again. At some point, quietly, you started wondering whether you're just someone who can't fix this.

You've tried at least 2 structured diet programs. None of them lasted more than 6 months.

Your PCP has run labs but hasn't offered anything more than lifestyle advice you already follow.

You've read about GLP-1 medications and want to understand whether they're right for your situation, specifically.

You want a real physician who knows your full history — not an app that mails you a prescription.

You want to know exactly what this costs before you call anyone.

"I've been doing everything right for years and my body just isn't responding the way it used to. At some point you have to accept that this is a medical thing, not a willpower thing."

That's the right conclusion.

Here's the medical explanation behind it.

The medical explanation

This probably isn't a discipline problem.
It's a biology problem.

Over 60% of people with persistent weight issues have a hormonal or metabolic component that diet and exercise alone can't address. For women over 40, hormonal shifts change how the body processes and stores fat in ways that calorie restriction doesn't reach.

GLP-1 medications — Wegovy, Zepbound, and the newer oral versions — work by changing how your body signals hunger and regulates blood sugar. They don't restrict what you eat. They change the biology that's been working against your effort.

A physician can run the right labs, review your full medical history, and tell you specifically whether this applies to you. That evaluation is what the program starts with.

One important thing: this program is for people who want to lose 15 to 40 pounds, get metabolic markers moving in the right direction, and maintain those results. It's physician-supervised medicine, built around your specific labs, history, and life.

The program

A physician who knows your history
before your first dose.

San Diego Docs is a concierge physician practice in San Diego. Dr. Danielle Fam and her team conducts the enrollment visit in person, reviews your complete medical history, and makes medication and program decisions based on your specific situation.

This is what separates physician-supervised care from a telehealth service. Dr. Fam's team knows your other medications, your family history, your lab trends, and your schedule. The plan she builds is yours. It is not a protocol that works the same for everyone.

"My goal at the enrollment visit is to understand the full picture. I review your labs, your history, your schedule, and what's actually been happening. The medication is one part of the plan. The rest is built around you."

Dr. Fam stays with you throughout the program. She doesn't hand you off to a patient coordinator after the first visit. If something isn't working or needs to be adjusted, you reach her.

What's included

More than a prescription. Your personalized plan.

FDA-approved GLP-1 medication

Wegovy, Zepbound, or oral semaglutide — selected based on your history and what makes sense for your situation. No compounded medications.

Physician oversight throughout

Dr. Fam monitors your progress, adjusts your dose based on how you respond, and is accessible between appointments if something comes up.

Prior authorization handled

Dr. Fam's team manages all prior authorization paperwork and manufacturer savings program enrollment. You don't spend hours on the phone with your insurance company.

Nutrition counseling

Practical guidance on eating patterns that support the medication, without a meal plan that requires you to reorganize your entire life.

Lab monitoring

Blood work reviewed at key intervals. Dr. Fam tracks your metabolic markers — blood sugar, cholesterol, and others — alongside your weight, so you see the full picture.

Meal delivery partnership

Optional access to a meal delivery service that works with the program. Useful during the first 2 to 3 months when establishing new eating patterns.

Transparent pricing

What you'll pay, all in.

Honest answers

The questions you're probably asking.

If you've researched weight loss programs before, you've probably been burned by vague answers. Here are the real ones.

  • What about nausea?

Nausea is real, especially in the first 2 to 4 weeks. Every patient in this program starts on a low dose and increases slowly. This is the standard titration protocol that significantly reduces side effects. If you're still struggling, Dr. Fam adjusts the approach. That's what having a physician involved actually means.

  • How much time does this take?

Your enrollment visit is about 60 minutes. The 30-day check-in is about 30 minutes. Monthly telehealth appointments fit into a lunch break. The program is designed for people who already have a full schedule. It doesn't ask you to show up every week.

  • What if I stop taking the medication?

Weight can return when GLP-1 medications are discontinued, particularly without maintained habit changes. The program addresses this directly — nutrition counseling, exercise recommendations, and the check-in structure are all designed to build patterns that support long-term results alongside the medication.

  • Do I qualify?

GLP-1 medications are FDA-approved for patients with a BMI of 27 or higher with a weight-related health condition, or BMI of 30 or higher. Dr. Fam reviews your specific labs and history at the enrollment visit to determine what's appropriate for you. If you're not sure, that's exactly what the consultation is for.

  • How is this different from Found or Ro?

Online-only services assign a telehealth provider who reviews a brief intake form and writes a prescription. Dr. Fam reviews your full medical history, knows your other medications, and adjusts your plan based on how you respond over time. That's a different kind of care. It's also why the program costs more than an app subscription.

  • What results can I realistically expect?

Most patients see measurable results within 4 to 8 weeks. Over 4 months, patients averaging consistent program participation lose 18 to 25 pounds. Results depend on your starting metabolic picture, which Dr. Fam assesses at enrollment. She'll give you a realistic expectation for your specific situation at that visit.

From a San Diego patient

A specific story from a patient
who sounds a lot like you.

"I'd tried Weight Watchers twice, Noom, and two rounds of cutting carbs on my own. I lost weight each time and gained it back. At 44 I finally went to a physician who actually looked at my labs and said, 'Here's what's happening metabolically.' It wasn't what I expected to hear. Four months later I've lost 22 pounds, my pre-diabetic markers improved at my annual physical, and for the first time in years I don't spend energy every day thinking about this."

Patient, Mission Hills. 44-year-old office administrator. 4 months in the program.
